ASSISTANCE
To apply for financial assistance through Dogslife, please complete the online form using the link within the "Approval Process" section below.
We want to ensure that grants are provided to individuals or families who are providing their dog with a loving home and who truly do not have the financial resources available at the time that their dog requires critical cancer-treatment. Therefore, we will require very detailed documentation that demonstrates your ownership history of your dog and your current hardship, including, but not limited to, proof of income and veterinary records.
Once your application, including all supporting documents, is received, we will begin our review process. Please note that we do not reimburse for treatments prior to an application being approved, including treatments that occur during the application review process. At this time we are only accepting applications for dogs being treated by board certified oncologists in the North Texas and Houston areas.
Approval Process
Your application will be reviewed and a background check will be performed. We may request additional documents and/or information during the review process.
If after review of your application, including supporting documents, the minimum requirements for assistance has been determined to have been met, we will contact your primary veterinarian, as well as your diagnosing board certified oncologist, to obtain additional information
After receiving all necessary documentation from both you and your veterinary providers, we will make a decision as to whether or not we will be able to provide financial assistance
